Error message when you try to save a record in Microsoft Dynamics CRM 4.0: "Object reference not set to an instance of an object"

When you get this error when creating or saving a record, this is because the last money field has been removed from the entity and the exchange rate plugin now can't find it.

resolution: add a new money field but dont add it to the form. publish. fixed.
